What Do I Need to Know to Enroll? 

Decide if you’d like to participate in the supplemental insurance, which types, and at what coverage level.

Enter your choices during your benefits enrollment event.

Designed as a complement to medical and disability coverage, the Accident, Hospital Indemnity, and Critical Illness Insurance plans through MetLife pay a lump-sum benefit payment if you have an accident, are admitted to a hospital, or experience one of the covered conditions.

Each plan pays a lump-sum benefit directly to you. This money can be used on anything you choose, from medical copays and deductibles to everyday expenses like mortgage payments and child care.

These optional benefits are employee-paid, and the cost will be deducted from your biweekly paycheck.

You are eligible to participate in Accident, Hospital Indemnity, and Critical Illness Insurance if you are actively working at least 20 hours per week at NortonLifeLock.

If both you and your spouse or domestic partner work for NortonLifeLock, you can each have up to one policy for Accident, Hospital Indemnity, and Critical Illness coverage. This means that one of you can elect each type of supplemental insurance for both of you, or you can each elect to cover only yourself. Additionally, only one of you can elect Accident, Hospital Indemnity, and Critical Illness Insurance for the same child/children.

Enrollment

You can elect coverage when you complete your benefits enrollment as a new hire or during Open Enrollment via benefitsolver.com (or through Benefitsolver on MyApps).

Accident Insurance Plan Details

Accident Insurance offers you financial protection when unexpected accidents happen. You receive a cash benefit if you’re injured and receive medical services, treatment, or hospitalization, provided you meet all the group policy and certificate requirements.

Both you and your dependents are eligible for coverage. 

Covered Conditions

  • Injuries: fractures, dislocations, concussions, lacerations, eye injuries, torn knee cartilage, ruptured discs, severe burns
  • Medical services and treatments: ambulance, emergency care, physical therapy, testing, medical appliances
  • Hospitalization: admission after an accident

Hospitalization $1,000 (per accident)
Daily Hospital Confinement $100 (up to 365 days)
Major Diagnostic Exam $150
Accident Follow-Up $50 (up to 2 times per accident, maximum of 6 per year)
Physical Therapy $35 (up to 10 visits)

Hospital Indemnity Insurance Plan Details

If you or a covered dependent is admitted to the hospital due to a covered accident, illness, or childbirth, Hospital Indemnity Insurance pays a cash benefit directly to you.

Hospital Admission $1,000 (up to 4 times a year)
ICU Admission $1,000 (up to 4 times a year, paid in addition to hospital admission)
Daily Hospital Limit $100 (up to 2 days)
Daily ICU Benefit $100 (up to 15 days, paid in addition to hospital admission)
Pregnancy Coverage  Normal and complications
Newborn Coverage Up to 31 days at no additional cost
Inpatient Mental Health and Substance Abuse Treatment Up to 30 days a year

Critical Illness Insurance Plan Details

If you experience one of the covered conditions within any category, you will receive a lump-sum benefit payment, provided you meet all the group policy and certificate requirements. You may receive more than one payment, depending on your covered conditions.

Calculating Your Monthly Rate

Please calculate your age as of December 31 this year. Rates are based on five-year age bands and will increase when a covered person reaches a new age band. Rates are subject to change. Be sure to read the Disclosure document (pdf) for details.
